Typically dental and vision care providers file insurance claims with Spirit Dental, or in this case, Meritain Health. If you must file your own claim forms, contact Spirit Dental for detailed instructions on how to fill out the claim form.

Spirit Dental offers customers the ability to search for dental and vision insurance from affiliated companies. Dental insurance policies are provided by Security Life or Symetra Life. Vision insurance is offered via Careington and EyeMed Vision Care Network. None of the policies purchased through the Spirit Dental website are underwritten by Spirit Dental.

Premium payments are required monthly. Dental insurance can be paired with vision insurance, but vision insurance does not appear to be available as a standalone insurance policy, according to the Spirit Dental website.

Coverage Details

There is very little information on Spirit Dental plans as the plan details are only available after visitors enter personal information. After entering information for the state of North Carolina, West Virginia and Nevada we realized different plans were available in different areas. The three plans we found were:

• Spirit Careington PPO: Patients are required to visit one of nearly 73,000 dentists in the network. A percentage of dental fees are paid by the insured. Coverage for basic, major and orthodontic care increases in years two and three of coverage.
• Spirit Indemnity: Patients have the option of choosing any dentist, but premiums are higher and no in-network discounts apply. Coverage for basic and major care increases in years two and three.
• Spirit EyeMed Vision: Coverage for eye exams, lenses, frames and contact lenses. Additional care may qualify for a percentage discount.
• Spirit Legacy Vision: Coverage for eye exams, lenses, frames and contact lenses. Additional care may qualify for a percentage discount.
